Collect the required identification, immunisation and proof of
address documents.
Submit Your Application
Return the completed application and supporting documents to the
school office for review.

What you need to provide
Please bring the following documents with the enrolment application.
Birth Certificate
Please provide your child’s birth certificate.
Immunisation History Statement
Provide your child’s current immunisation history statement from
the Australian Immunisation Register.
Proof of Home Address
Provide proof of your home address, such as a utility bill or
driver’s licence.
